Request for documents
If you wish to obtain information on someone else’s records over whom you have no legal authority, you must have a power of attorney signed by the person concerned (or his/her legal representatives). Otherwise, the health insurance company will refuse to give out information, in accordance with the Data Protection Law.
You can find a pre-filled power of attorney on our website. Have it completed and signed by the person concerned by your request for information, and return the document to us.

For clients travelling to Russia, Mundo supplemental insurance is required, unless you have similar coverage with another insurance company.

However, you will have to ask Groupe Mutuel directly (by post, email or phone) for certain specific documents, such as direct debit forms (LSV).
